The ruling of the Board of Education is that only those children who will be five years old on or prior to October 1, 2010 will be admitted to kindergarten. For registration, the following will be needed: |

| 4. |
Two proofs of residency that the child and parent/guardian reside in the school district. One required proof should be a copy of a mortgage or lease agreement and recent utility bill. |

To comply with New Jersey State Laws, we need all dates (month, day, year) for the following immunizations | |

Kindergarten Vaccination |
| 1. Diphtheria and Tetanus Toxoids and Pertussis (DTP) Vaccine - A minimum of four doses of DPT are required. One dose must have been administered on or after the fourth birthday. |
| 2. Poliovirus Vaccine - A minimum of three doses of polio vaccine is required, provided at least one dose is given on or after the fourth birthday. |
| 3. Measles Vaccine - Two doses of a measles-containing vaccine given after the first birthday are required. Documented laboratory evidence of measles immunity is acceptable. |
| 4. Rubella Vaccine - Two doses of live vaccine given on or after the first birthday or laboratory evidence of immunity is required. |
| 5. Mumps Vaccine - Children in schools or child care centers must have one dose of mumps vaccine administered on or after the first birthday. Documented laboratory evidence of mumps immunity is accepted. |
| 6. Mantoux - The tuberculin skin test is recommended by the State Department of Health and Education. When your child has a physical exam, and needs a tuberculin test, please request your physician to use the Mantoux Test.. |
| 7. Hepatitis B Vaccine - Three doses for all kindergarten students. |

8. Varicella - One dose on or after first birth date or date of disease (written statement from physician or parent) 9. Tdap -1 dose for students entering Grade 6 and born after January 1, 1997(given no earlier than the 10th birthday. 10. Pneumoccal Conjugate Vaccine - 1 dose on or after first birthday for age 12-59 months enrolled in preschool. 11. Influenza - 1 dose between September 1 and December 31 of each year for age 12-59 months. 12. Meningococcal Conjugate Vaccine - 1 dose for students entering Grade 6 and born after January 1, 1997. |
Parents must present evidence at time of registration of the required immunizations, or a statement from the physician/health clinic that the immunizations are being administered. Admission to school in September will necessitate these immunizations. Also, the medical examination forms must be completed and returned to the school nurse prior to the first day of school. Your child does not need to attend registration, unless English is not the primary language. He/She will be invited to an orientation prior to the first day if the school year.
